Impact of chronic wound fluid on a non-healing wound
Exudate is a normal feature of wound healing produced in the inflammatory and proliferative phases, but exudate form chronic non-healing wounds can have a deleterious effect on the activity of certain cells and growth factors involved in the healing cascade. It contains a significant number of growth inhibitory agents such as matrix metalloproteinases, which if they remain in direct contact with the wound (as is the case with more typical ‘interactive’ dressings) will in fact inhibit healing. In a study by Bucalo, Eaglestein and Falanga chronic wound fluid was shown to inhibit the proliferation of human dermal fibroblasts and failed to stimulate the proliferation of microvascular endothelial cells and keratinocytes (3). In addition, wound exudate increases the risk of infection if it strikes through primary and secondary dressings and can be a portal for the access of pathogens. Hence the retention of exudate close to the wound surface can limit the usefulness of occlusive dressings in chronic wounds.
